We connect people with two types of licensed professionals depending on their needs: licensed care coordinators for Remote Patient Monitoring (RPM) programs, and licensed insurance professionals for Medicare plan enrollment questions. Our referral service is always free to you — professionals in our network are compensated by the programs and carriers they work with, never by you directly.
Medicare Part B may cover FDA-approved health monitoring devices for qualifying seniors with chronic conditions. We connect eligible individuals with licensed care coordinators who work with RPM program operators to determine eligibility, coordinate physician authorization, and facilitate device enrollment.
We do not: provide medical care, prescribe devices, diagnose conditions, or guarantee Medicare coverage or device approval. Eligibility depends on your specific Medicare plan, diagnosis, and physician authorization.
Navigating Medicare enrollment — especially for the first time — can be complex. We connect seniors with licensed insurance professionals who can explain Medicare Advantage, Supplement (Medigap), and Part D plan options available in your area.
We do not: offer every plan available in your area. Please contact Medicare.gov or 1-800-MEDICARE for a complete list of plans. Licensed professionals in our network are contracted with various Medicare carriers.

For RPM: a care coordinator will review your Medicare plan and chronic conditions, and coordinate with your physician to determine which devices, if any, you may qualify for. For insurance: a licensed professional will review plans available in your zip code based on your doctors, prescriptions, and budget — at no cost to you.

Care coordinators and insurance professionals in our network hold active licenses appropriate to their role. Insurance professionals are licensed in the states where they operate.
